Referrals & Support
Family Medical Center is a medical home for your personal and family medical needs. We manage and integrate your direct and referred care.
Referrals
Referrals to specialty providers for initial evaluation and continued care are handled by the office and often require an appointment. Some referrals may require you to call your insurer (or check their website) for a list of covered in-network providers.
Imaging, lab testing, and other ancillary services may be ordered as part of your care plan — check with your insurer for preferred facilities.
Prescription Refills
Prescription refills are generally handled at appointments and authorized or discussed during the visit. For medications prescribed by Family Medical Center, contact your pharmacy to request a refill; refills are authorized per office policy.
Patient Portal
Our medical records portal software is being improved. A link to the portal will be made available here when complete.
Office Forms
Complete forms before your visit. The following forms are available at check-in:
- New Patient Packet
- Medical Records Request / Transfer
- Sports / School Physical
All office forms are available at check-in. Ask our front desk or call to have forms sent to you.
